May be you are trying to install some different printer driver or for the different OS.

Can you please upload a screenshot? It should not be there after formatting. Have you installed any addon extension in chrome? Screenshot can help a lot.
Are you printing from Notepad or Microsoft Word or name the application if any other. Also please check the ink level of your cartridges it may be possible that one of your cartridge get empty. Another possible reason is make sure that you are not printing in White Color.

It could be the problem of Time & Date. Please check your computer’s time & date and correct that then try again.
